How to unlock the Well-Prepared achievement in Dragon Age Inquisition - Definitive Guide

Once you have completed the prologue you will be in Haven. The first thing you will have to do is go to the chantrey to speak with Cassandra and you'll form the inquisition.
Once this is done the area will open up. You should now have a quest to speak to people around town called Haven's best and brightest.
You now want to go out the front gate of haven and go left on the path where you'll see the blacksmith. If you're struggling to find him then open your quest map and it is the most north west diamond. Speak to Harritt and then use the table to make some armour or weapons. Ensure that you don't make an add on, for example boots, hilt or arms as they won't count. You need an item that will go in a slot, so any weapon is best (Thanks Elite). If you don't have the materials then there are a few in the haven zone as well as a couple of crates in the forge.

I believe if you then speak with the Smith again he'll give you an upgrade recipe which will let you do

I had some issues getting this to unlock, made some cloth boots and it didn't work (twice).
Made a dagger and it worked.

Ah, I realize now because boots in this game are technically an 'addon' for armor. So yeah. I'd go with a dagger as it would probably be the cheapest resource wise.

Shortly after the tutorial is completed, you will be given a quest that includes going to speak with the smith. Once you arrive, the smith will update your quest and instruct you to loot one of the crates beside him for crafting materials. After doing so, turn and face inwards to the smithing area and the table closest to you will say “Craft Armor.” Activate it with  and follow the prompts to craft your first piece of armor.

You can craft and modify weapons and armor at a Blacksmith. This can be done in Haven at the Blacksmith who's right outside the village walls, or if you're in Skyhold in the Undercroft which is the door on the right of your Throne. You can also craft and modify weapons in any of the three keeps.

In order to craft a weapon or armor, you'll need a schematic. These can be purchased from various merchants, or found throughout the game. Any schematic will do. You'll also need crafting materials. Crafting materials can also be bought, but are mostly found throughout the game. Many of the schematics can use many of the countless materials.

Go to any crafting table and select any of the schematics you have. You'll see a few upgrade slots in the form of hexagons. Select any of the slots and it will show you a list of the materials you have that can be used, and how many you have and need. Each material adds different stats. Pick any materials to your liking. Do this for every slot and press afterwards to craft the item.
