Also, dont forget to talk with your companions at the hub area regularly. They always have codex entries that will shed more light on DA lore.
Other than that, make sure you take your time. These worlds are massive, and only a careful inquisitor with patience can unlock it all. Follow these tips, and youll have this cheevo popped within a few hours of play (maybe less). Enjoy!
- Talk and explore unique conversation options with people;
- Gather plants, minerals and create new unique consumable items;
- Kill wildlife and different enemy types in general;
- Find and interact with certain objects, such as reading wall signs with the button.
You don't have to play the whole game focusing on finding them (the game has more than just 250 pieces of codex), but it helps to unlock the achievement sooner if you take advantage of all the early opportunities.
If your playstyle is as thorough as mine, you will be able to unlock this achievement on your first visit to Val Royeaux. That means I had only explored 5 out of the 20 areas of the game prior to that point.
Note that when I did that, I've intentionally beaten all available missions in those 5 areas before trying to progress with the Main Story. In other words, I had finished and explored the maps as much as possible in the following locations:
- Haven (explored the whole map and talked to everybody there is to talk to).
- Hinterlands (you won't be able to enter Redcliff yet and except for the upper right corner of the map where there is a High Dragon and which I decided to avoid since I'm playing on Nightmare difficulty, I've done all the other quests).
- Fallow Mire (did everything there is to do there, explored the whole map).
- Storm Coast (explored the whole map in my first time there, except for all that is behind the sealed door).
- Forbidden Oasis (explored as much as I could and just didn't get through all of the doors at the temple because I didn't have enough shards).

Codex entries come from most items you can Read or Examine in the world. This includes the first time you pick up a new type of weapon, herb, crafting material, or even the people you meet in the world. Anytime you interact with items of this nature you will see on the left hand side of your screen an entry for "Codex Unlocked".
While 250 entries sounds like a lot there are WAY more than that provided you don't mind looking for them. I myself unlocked this trophy only 22 hours into the game just after reaching Skyhold for the first time.
You will come across many ways of discovering a codex entry, including (but not limited to): interacting with notes, reading books, extracting information through conversation, exploring, etc. Once a codex entry has been unlocked, you will receive a notification in the bottom left side of your screen. As there are a large abundance of codex entries to be found, going out of your way for this is not necessary; simply interact with everything available and choose to extract information from as many conversations as possible. This will easily come with time. You must collect 250 of these entries to unlock the achievement.

Pretty much everything you do will unlock Codex entries. Characters you meet, obtaining materials, enemies you defeat, places you visit, loot, letters and books and all kinds of stuff you can examine. The list goes on. This will come pretty quickly, don't worry about it.
