How to unlock the Marked for Greatness achievement in Dragon Age: Inquisition - Definitive Guide

Just close every rift you see, and eventually you will get this achievement, there are more than 75 rifts throughout the game, and luckily you get quests to close them, which shows you the location of the rifts.
Here are just a few general tips that made my life a bit easier for finding, and getting to them.
-Set up as many camps as possible, save lots of time in case you missed one, and it is one the other side of the world.
-Use mounts, some areas are huge, can take several minutes off the journey.
-If you see one, and it is inconveniently far from any camps, or fast travels deal with it, if possible, saves you the headache later.

Can you come back after the end of the story to finish the rifts off?

Those lesser terrors are no joke on Nightmare, they just keep spamming that move that knocks my party down, and kill me off before I have a chance lol. Guess I am going to need to level up a bit more.

Seal 75 rifts in a single playthrough.

There is noting wrong with cheeseninja123's solution, and all of his tips are going to help you complete the achievement. But I thought this would come in handy too for anyone working on this achievement.

First, fade rifts are marked on your maps as a Black Flag with a star cut out of the middle. If you select the fade rift as your primary quest, it will place an Inquisition eye marker over each rift associated with that quest. In MOST areas, the fade rifts are spread across multiple quests. One quest may require you to close 2 rifts in one area of the region, while another may require you to close 4 rifts in a different area. Make sure you do all of the quests!

Second, here is how many fade rifts are in each area, listed in the order of area difficulty/required level, AND how many quests the rifts are spread across:

Hinterlands - 12 (4 quests)
Storm Coast - 3 (2 quests)
Fallow Mire - 2 (1 quest)
Crestwood - 4 (2 quests)
Forbidden Oasis - 5 (2 quests)
Western Approach - 8 (4 quests)
Exalted Plains - 11 (2 quests?)
Emerald Graves - 11 (4 quests)
Emprise du Lion - 6 (3 quests)
Hissing Wastes - 12 (3 quests)

You will notice if you add all of these up, it only comes to 74 while the achievement requires to get 75. Well, any rifts that you close via the main story line or side quests are also included in this achievement, so that is how you obtain 75+ closings. So you must progress the story or do side quests to ensure that you can get this achievement.

This achievement will also help you with:

As all of the enemies that spawn from fade rifts are demons.

The achievement is technically missable if you do not choose to go out and complete the side quests to close the rifts. Without closing the rifts not related to the main story, you will not even get close to closing 75 rifts. So, if you only played through the story and did nothing else then you would miss this achievement.

Yeah but can't you come back to finish it after you finish the story? As I'm playing on nightmare, I want to finish story, switch to normal and mop up....

Yeah, to my knowledge you absolutely can. I don't think it should matter when you seal the rifts - before or after the main story is completed. But it is still a "missable" achievement because you won't get it just by completing the story. That is my only explanation.

To get this trophy, you just need to close 75 rifts across the larger map areas.

There are more than 75 in the game - I think there are 87 in total as the trophy dropped for me after closing the last rift in Emprise du Lion, and at that point I'd closed every rift in every area except the Hissing Wastes which has a further 12.

Note that if you are also attempting to get the Demonslayer trophy (for killing 1000 demons in one play through), you may not wish to close every rift until you've earned that trophy - attempting rifts, getting a few kills, then abandoning the rift by running away is a good way to farm kills.

You don't need to kill 1000 demons in ONE playthrough for Demonslayer trophy.
It stacked between different character you made.

You'll get this trophy in one playthrough if you only do half the rifts anyway.
There's tons of demons during "here lies the abyss" and the first boss in The Winter Palace spawns a ton.
MisterJohn is also correct that although you'll unlock demonslayer by proxy, it doesn't have to be done in one run.

You will come across rifts quite regularly, marked as the same icon displayed when an autosave occurs. For a seal to occur, you must first defeat all demons in and around the immediate area. During the fight, you may target the rift and press  to disrupt it multiple times, stunning the demons you are engaged with and making the fight somewhat easier. Once all the demons are defeated, target the rift and press  again. Your Herald will dispose of the rift with their mark of Andraste. Seal a total of 75 rifts to unlock the achievement.

There are more than 75 rifts in the game. Keep an eye open in every area while you progress other achievement requirements.

Rifts are explained to you in the prologue of the game. Whenever you get close to a rift, enemies will spawn. Defeat these enemies and the rift will lose health. Defeat all enemies and you'll be able to close a rift. During the fight you can also disrupt the rift, this will damage and stun all demons still alive. Be careful though, as you're completely vulnerable while doing so. A great tactic is to bring a Mage with Dispel or Spell Purge. Use Dispel/Spell Purge on an enemy that's about to spawn to instantly destroy it. Do this before the dot on the minimap turns red though, otherwise it won't work.

Throughout the game you'll come across Rifts. Rifts that you find in any area will appear on your map, and any Rift tied to it will also appear on your map. This will also start a quest tied to those Rifts. There are multiple of these in each area. There are a total of 70 Rifts you can find like this. There are also Rifts that you need to close during Story and Side quests. In order to obtain this trophy you'll have to close almost every Rift of those 70 if you've finished the story.
