How to unlock the A Dark Promise achievement in Dragon Age: Origins - Definitive Guide
I don't think you can miss the offer to get this achievement, though I could be wrong (I am 99% sure I am right though). If you're chose a female as your character, you can still get this achievement. Morrigan will still offer you the ritual. There are two other members whom she can perform this ritual with (one being more vague than the other but both unlikely in the grand scheme of things). So just agree to Morrigan's offer and you will pop this achievement.
Additional note: If you don't want this to be your ending, just save prior to agreeing to Morrigan's request. Once you complete the game, reload your save prior to speaking with Morrigan and decline her offer.
If you accept her offer then you sleep with her (or, if you are female, you must convince Alistair to sleep with her). Once you fight through the battle and slay the dragon, you'll receive this achievement.
If you want to get the Ultimate Sacrifice achievement, save the game before you make any agreement with Morrigan, play through the battle to get this achievement, then reload and play again. It took me about an hour to play through the final battle.
After "The Landsmeet" ends, travel to Redcliffe. You will find out what the fate of the Grey Wardens is. Morrigan will offer you the chance of going through a dark ritual the night before the battle. Accept it. Then complete "The Final Battle". There's nothing special to "The Final Battle" other than surviving waves upon waves of Darkspawn.

One Playthrough Method: After discussing what to do in Redcliffe, make an extra save. In one, refuse Morrigan, complete the game, and then reload. Accept Morrigan's offer.
The first thing you need to do is upgrade the Herbalist skill of your character or party member to the maximum 4th rank.
I advise you to upgrade this skill from Morrigan. She will become our party member at level 7 and will already have the 2nd rank of herbalist out of 4.
This means that she can get rank 4 herbalist at level 12 if she invests in herbalist every time she has the opportunity to increase her skills.
As soon as you have upgraded your herbalist skill to the required rank, your path lies in Denerim, where in the “Miracles of Thedas” store for an immodest fee ( 11 gold 17 silver 50 copper ) you must buy a recipe for a Strong lyrium potion.
To create one potent lyrium potion you will need: 4 portions of lyrium dust, 1 flask, 2 sublimation reagents and 2 concentrating reagents.
All these ingredients need to be purchased from certain merchants:
1. Lyrium dust is sold by the quartermaster in the Circle Tower.
2. All other ingredients are sold by the innkeeper from the “Bitten Nobleman” tavern in the Trade District of Denerim.
The cost of purchasing all these ingredients to create one powerful lyrium potion
will be: 1 gold 0 silver 31 copper - (1.031)
And you can sell the finished potion for: 1 gold 25 silver 0 copper – (1.25)
We calculate the profit: 1.25 - 1.031 = 0.219 which equals – 21 silver 90 copper net profit from the sale of each potion.
And when selling 99 potions, the profit will already be 21 gold 68 silver 10 copper.
When you make a potion, you should have enough free space in your backpack.
The more slots you free up, the more ingredients you can buy at one time, which means the less you have to run around to merchants.
