How to unlock the Blind Vengeance achievement in Dragon Age: Origins - Definitive Guide
The achievement pops at the end of the obligatory cut-scene.
This is another along the main quest. You'll be asked to go and speak to Mervis in Amaranthine about merchant caravans disappearing. He'll direct you to the Wending Wood. After exploring, you'll meet Velanna. She will tell you who is behind the caravan attacks. Find the sole militia survivor after that. After encountering Velanna again, head to the Darkspawn Camp and then head into the Silverite Mine. Once inside, you'll see The Architect for the first time. You'll be imprisoned and all of your items will be taken away. After escaping from your cell, you'll have to fight, but without your weapons and armor. Pick up what you can on the way. Each character's equipment will be worn by an "Experimental Subject". Kill them to get your gear back. After you have all of it, your final fight will be against two dragon thralls. After a few hits, they'll both fly to another spot in the room.

In a location called Vending Forest , there is a silverite mine (mine) in the very north-central part of the map.
You will get into it when you talk to an angry elf named Velanna.
After you talk to her, she will join your party and invite you to go to the silverite mine to find her sister.
So, in this mine, there is a very unpleasant “glitch” due to which your character and squad companions may forever lose their uniform, so before going into the mine , be sure to save the game!
The Architect, an intelligent Spawn of Darkness, will greet you in the mine.
He will be able to subject you to magical effects, as a result of which you will find yourself in a dungeon without equipment or uniform.
If your character and companions find themselves dressed in simple clothes with an empty backpack, then everything is fine.
But if you still have some of your old property, or your character is not wearing simple clothes, then you have a glitch and you will have to reboot, because in this case you will never see your old uniform again.
Also, a glitch can be identified during the Architect’s speech: if during a conversation a room with cells is visible behind him, everything is fine, but if he speaks against a black background, then this is a glitch and you need to reload the game save, which you need to do before entering into the mine.
Most likely, this glitch occurs if your character or one of your companions is wearing items from the downloadable content - Duncan's Sword, Duncan's Dagger, Bregan's Bow, Kadash's Tall Boots, Provocateur's Battle Robe, Belt of Forbidden Secrets.
To get around this glitch, remove all the equipment from your character and squad companions , placing it in your backpack inventory and go “naked” into the mine.
Your backpack with all the equipment, uniforms and weapons will still be taken away at the very beginning.
But then, as you move through the mine, you will come across enemies called “Experimental Objects”; they will divide all your equipment among themselves.
By killing them, you will return your uniform and weapons.
Every time such enemies appear on the horizon, your companions will “comment” on their appearance.
At the end, when you reach the Qunari trader named Armaas, it will mean you are almost at the exit.
In the chest not far from it, there should be all your other things - all the rest of your inventory.
Remember, after you leave the mine, you will not be able to return to it again, so explore the entire mine at once to the end!
I personally got burned by this glitch and didn’t know how to fix it, so I had to buy new uniforms and weapons for my squad again.
So keep this in mind and always save the game more often, otherwise at any moment, everything may not go according to plan.
