This trophy is called "Your Suffering Starts Now" and it requires you to upgrade a skill to its max level of +20.
The earliest you can earn this is at Level 21 with your Raider. This is because you begin at Level 1 and need to level up 20 times to earn 20 skill points.
To level up a skill for your Raider, press the Options Button while not in a match then press R1 to view all the Raiders. Scroll through your Raiders and select which one you've leveled up. From there you'll see the skill list and you can scroll through the skills for each stage of the Raider. Highlight a skill and press and hold the X Button to spend the skill point and add +1 to the skill.
Repeat 19 more times on the same exact skill to unlock the trophy.

Unlike Survivors who use Spirit given after matches to level up skills, each Raider has their own level which grants them skill points. Invest 20 points in a single skill on a Raider of your choice, which can be done as soon as Lv21.
