How to unlock the The Power of Super Saiyan Blue and Kaioken! achievement in DRAGON BALL XENOVERSE 2 - Definitive Guide
The achievement doesn't specify, but you need to talk to a man named Oshi after reaching level 99 in order to unlock this achievement. He's standing on the path that leads up to the Dragonball pedestal and is gold coloured, making him easy to spot.
Edit: It should also be mentioned that once you reach level 80, you'll need to visit Guru on Namek to increase your level cap to 85, and then again every 5 levels until you reach level 95, where he will increase it once more to level 99.

Are you allowed to level up via the dragon balls or does the last level have to be done normally?

You can use the Dragonballs, it's how I got from 86 to 99.

I make a small clarification to level up quickly is convenient to use the Dragon Balls.

To level up to 99 you can either do the traditional way of completing missions and what not or pay your way up using the Namekian 'Tosok' with a mixture of Zeni (Levels 1-80) and TP Medals (Levels 81-99). I did both and in my video I show you me paying to level up from 98 to 99. Then you must go and visit the gold-armoured Oshi who is just down the walkway from the DRAGON BALL PEDESTAL.

When this was released people needed to grind dragon balls to level up; however, a later update introduced a Namekian NPC named Tosok in the south of the map near the item shop.
If you haven't played for a while, you will likely find system gifts of TP medals waiting for you that you can use towards this. If you wish to save medals you could upload your save to Cloud Storage/USB, spend the Zeni/Medals, and then download your file to get the Zeni/Medals back. The amount of TP medals required to get from 80 to 99 is below 1000 so it shouldn't be an issue to get, though I'd use the Dragon Ball method if you want to save the TP medals for the Hero Colosseum.
If you wish to do it via Dragon Balls, then farm Mission 23 (The Explosion of Namek), quickly defeat Frieza and then the time patrollers that show up. Each patroller can drop a ball so it is possible to get 3 in one mission if you're really lucky (had it happen for me once, very rare). When wishing you'll have two options: "I want to Grow" (gives 1 level up) and "I want to Grow more" (if you had Guru power up your wishes). Grow more grants 3 level ups; however, it won't allow you to pass your cap, so if you are 2 levels away then it'll be unavailable, so you'll be looking at alternating between the two wishes. If you hit the caps below the 99 one, then go talk to Guru so he can unlock it for you.
For the trophy to actually pop, you have to talk to Oshi who is north of the Dragon Pedestal where you use the Dragon Balls at the bottom of the steps.
