As with most RPG's, once you finish the story you'll be given the opportunity to "free roam". Allowing you to mop up achievements like this. You'll even gain access back to locations you've only been able to visit briefly during the story.
Press start and press "y". This will open up your encyclopedia.
There are a total of 938 possible entries that you can find throughout the game. Some you'll need to explore to find, and others can be obtained through normal progression of the story. I personally explored quite heavily and was able to obtain this achievement by the beginning of the android saga. By the end of the game I had obtained 839. Pick up everything you find, complete side missions, explore, etc. This achievement should pop for everyone without any issue.

The Z encyclopedia is a feature that contain almost every information of all the world of Dragon Ball. To access the Encyclopedia, just press 
in the pause menu and chose from the summary the informations you want to know.
To unlock this trophy, you need to unlock 400 entries in the encyclopedia. There are different ways to unlock entries:

The, Pants, Party
This achievement is essentially story-related. There are nearly 1000 entries in the Encyclopedia and over 700 can not be missed as you play through the story. Be sure to visit the Encyclopedia from time to time to get your rewards, namely the D Medals.
