I think this is bugged for me :( I changed skin and hair color and nothing popped.

Edit: Tried it on PC and it unlocked after two minutes.

Mine also didn’t pop on time. I loaded it up on cloud and it popped shortly after changing again.

You can actually get this slightly earlier. I'm not sure what triggers the letters in the mailbox to come down, but I'd done both of the side islands when I noticed them. You get a letter which unlocks the Angler's Isle, which has a dressing table on that you can use smile

Later in the story (during Khrumbul-Dun), you'll unlock the ability to craft a dressing table. Once you do so, use the table and select your gender. Now simply change something like your eye colour and exit the table. This trophy will unlock. Changing your equipment at the table does not unlock the trophy.

The trophy is glitchy. I changed my appearance twice already on the table that I found on the Island of Awakening - they never gave me the trophy. You probably need to build a table yourself and change the appearance on it.

My trophy dropped when I completely changed all the cells (weapon, armor, shield, hammer, headdress and three accessories).
