How to unlock the If At First You Don't Succeed achievement in DRAGON QUEST XI: Echoes of an Elusive Age - Definitive Guide

Not only can you create items at the Fun-Size Forge but you can also rework gear that you already own into +1, +2 and +3 variants to increase it's stats and make it more effective. Instead of ingredients, reworking items puts use to the Perfectionist Pearls that you have gained from successfully crafting items. Gear will require different amounts of Pearls dependent on the rank and difficulty of the piece of gear so you won't be able to tell how many are needed until you are in the Rework Item menu of the Fun-Size Forge.

To unlock this trophy you will need to successfully rework items ten times. Reworking an item works exactly the same as crafting an item in terms of the forging mini game. You will need to be more mindful when attempting to hit as close to the middle of the "sweet spot" indicator as possible on all segments as this is what determines the strength of your upgrade. Likewise, if you fail too many segments you will fail the upgrade completely and lose all Perfectionist Pearls used in the attempt. Your original item will be returned to you and you can rework gear an unlimited amount of times until it reaches +3 so this isn't as severe a handicap as it may first seem. Pearls are easily re-obtained and can even be purchased from a vendor later in the game for 100G a piece.

It may not seem entirely obvious to some, like me (I realized it at 20+ hours), that any equipment can be recrafted, including those purchased in the store, those dropped from monsters, and characters’ own equipment. So you don’t have to put it off until later, but immediately improve any purchased/selected item.
