Each big city like Bakbattahl or Vernworth will have a morgue wich you can recognise by the coffin icon.
When an NPC dies they will appear in the morgue after a day or two. You might have to search their coffin on the different floors.
To get this achievement, you can walk up to a coffin inside the morgue and use a wakestone to revive the NPC.
I made a backup save at an inn, killed a couple of NPCs, waited a couple of days and went into the morgue to avoid wasting the wakestone. Afterward I reloaded my save.

I’ve done it twice and the achievement is not popping for me.

Save this until you are doing the spellbound quest as there is a good chance you will need to go to the morgue to progress it as one of the grandparents will be dead. I've seen a lot of people have to do it as well as myself.
