This is a missable false ending during your fight with the Seneschal in the quest "Final Judgement." After the first two fights, you will be placed in a long linear vortex-like hallway with npc's from the game in front of you. For this achievement you need to turn around and walk away from them. You will get a cutscene, a game over, and the achievement.

I think it's important to note that when you do this, you will be put on the beach in Cassardis.. just walk toward the gate, toward Quina and Aldos... then the cutscene will happen, a game over, the achievement and the option to retry to continue where you were before you turned around in the hallway.

This may be a bit buggy. I’ve gone through it about 3 times and it hasn’t popped. I’ll update if it pops late.
