You can do this achievement pretty early. If you have a weapon just drop the weapon and pick it up, it won't equip it. While having only your fists go to an island and look for a graunt (the small ones) and hit him. To make it easier try to fix it to a wall or something like that so he can't walk away. It may take some hits since you'll only make 1 dmg per hit.

Note, you have to kill them completely empty handed. I tried whittling them down with a weapon and then killing with my fists and it didn't unlock.
