The "Mask of vampire" can be obtained by collecting cats, every sixth one will give you an item. Items you can receive are Pickaxe, Tank Armor, Boots Of Dexterity, Vampire Mask, Monocle Of Goo, and Monocle Of Inversion.

You don't get an item for collecting the 24th cat, so there is no point in collecting more than 18, unless your aiming to get the achievement for collecting 24 cats. The item isn't automictically added to your inventory when clicking on the sixth cat, it will be on the ground. Once in your inventory, equip the item and the achievement will unlock.

So each game you have 3 chances of receiving it, with each chance being a 1/6 chance of being the "Mask of vampire". The cats are in different locations every time, except for the one in the village that is located to the right of the starting location, if you turn right as you walk out and open the first door on your right. There is a cat on every level of a tower/building.

Mask of Vampire is a “heavily rare” drop that can be given for collecting the cats in the game. You will get a item on your 6th, 12th, and 18th cat collected. The Mask of Vampire is one possible item drop from RNG & only way to get.
