How to unlock the Beating the One-Eyed Worm achievement in Duke Nukem Forever - Definitive Guide

Story-Related, Can't be missed: Unlocks after defeating the Energy Leech Boss at the end of the level "Blowin' the Dam"

Hahaha another masturbation joke! But seriously this fight is not that bad--especially after that Octoking *shiver*

The arena is set-up such that there is an ammo crate at left and right and bubble streams in the middle area--yes this means the entire battle is underwater. Basically you want to strafe left and right firing rockets and collecting bubbles, making sure to reload your ammo each pass. 12 rockets (direct hits) will take him down--then refill your bubbles and place the mine in his gill for the kill.

Video below.

Strafing strategy didn't work for me -- the leech too often hit the same place twice, and with deprived mobility, I couldn't avoid it.

Then I read a comment somewhere in the internets to camp near the right hand side of the arena, near the air vent and ammo crate. Allegedly the leech couldn't hit there, but it can... but also for some reason it wouldn't do those multiple hits and blam, I got it. Played on normal.

Note: If you're looking for a super easy way to beat this boss, skip the next paragraph to see a method that should one-hit kill it.

This boss is much easier than the last boss. Since there are no other enemies trying to kill you, this boss battle should be pretty easy for you. The only thing that makes this guy hard is that you're fighting him underwater. This makes you have to swim over those air vents in order to give Duke his oxygen so he won't die. The boss has three attacks: Shooting some sort of electric ball at you, trying to smash you, and trying to suck you into his giant mouth. All of these attacks are easily avoided by strafing to the left and right nonstop, except when you have to pick up ammo. While swimming, shoot with the RPG found next to the valve you had to turn, and when you get out of ammo, swim to one of the two EDF Ammo Crates located on the right and left side of the area. Keep doing this and you will soon be able to swim up to it and insert a bomb in it using first and then pressing repeatedly.

You can also one-hit kill the boss using the following method:

Well, basically a 1 hit kill. What you'll need to do is swim up the the boss and pull the trigger. You will get frozen, your ego will drop to nothing and your screen will be flashing red but you will see the the Energy Leech's health will completely deplete while you are frozen. Once you unfreeze, all you need to do is switch to a RPG and shoot him once and that will put his health in the red flashing state so you can start the QTE event to finish him off.

Just something I stumbled upon last night completely by accident and I reloaded to verify it works everytime and wasnt just a glitch. Hopefully this is helpful for those getting really frustrated at this boss fight. If you dont have the Freeze Ray, you can get one in the previous chapter.
