How to unlock the Not a scratch achievement in Dungeons 3 - Definitive Guide
Simply keep Thalya separated from your main horde to start. Move the horde all together and engage enemies as you find them. While my horde was fighting, I moved Thalya a bit closer but kept her out of the fight. The hardest part of this achievement is when you first engage Thanos. After the cut-scene, Thanos will put a red circle on the ground where he is casting his AOE fire spell. Immediately move your horde out of the way. Target Thalya individually and move her to a safe spot. Carefully hold "A" and select the rest of your horde to engage Thanos. Once they are engaged, target Thayla individually again and be ready to move her in case you see the red circle appear under her again.
Once this event is done, ***SAVE***. Move her up through the graveyard, and watch for more red circles on the ground so you can avoid them. After a while proceeding through the graveyard, you will encounter the Undead Golem. After this cut-scene, ***SAVE***. You will need to move sort of quickly at this point as a wall of fire will follow behind. You cannot leave Thayla too far behind or she dies in the fire. You must move up a bit, but as soon as you see the next group of mobs, move the Golem up alone and engage. Once engaged, you can select Thayla and have her use her magic on the closest enemy. This fight takes just long enough for the wall of fire to move up on you. If the caster heals the enemy too much, you might get caught from behind. Simply reload save if you die. Once you kill these 3, move up further. Continue with this strategy for the next group as well. Wait for the Golem to engage enemies before you have Thayla move up to use spells from a distance.
Once the next group has died as well, ***SAVE***
The next part involves a barricade you need to destroy to get into the town and the next set of enemies. Again leave Thayla behind and select the Golem to attack the barricade. During this fight, Thayla kept wanting to move up and engage on her own. I had to keep her targeted and moving her back when she tried. Once the Golem finishes off this group, you can move him up and get the health potion. Move Thayla into the town, but keep her back while you move the Golem up to the final group of enemies. Once it is engaged, you can have her cast spells from a distance to assist, but this is the last fight.

Done in the "Baptism of Fire" level. Probably one of the tougher trophies in the game, not because it's difficult but because of the clunky controls and auto-attack mechanics, making controlling Thalya an annoyingly challenging task, especially to keep her uninjured throughout this level. Use the Grave Golem to tank all damage, and make sure to keep Thalya on a short leash as auto-attack may cause her to rush forward and take damage as she walks faster than the Grave Golem.
In the beginning area, there's an archer ambush which may target Thalya even if you're surrounded by your creatures. Your best bet is to keep Thalya at the beginning area while your creatures go ahead. Your creatures will be more than enough to fight through. Once you reach the boss at the middle of the map, Thalya will teleport to the event and allow you to proceed without damage. Later on when Thalya is alone with the Grave Golem, pay particular attention to Novices who cast an area of effect light blast spell which may catch Thalya if she's too near.
It's best to keep Thalya out of battle for the 1st 3rd of the level. At the last 3rd of the level keep Thalya on a tight leash. If you complete the level with Thalya staying at 100% health throughout, the trophy is yours.
