First off you will need a wrench which is found more prominently early in the game but you can find a few in car shops and for sale in stores. It is best to just hang on to the first wrench you find until you have the skills necessary to pull the achievement off. The skills you need is Vault which is on the agility tree and can be first attained on level 4. The second is Drop Attack at level 10 on the power tree. Now that you are all set up equip your wrench and run at a group of zombies (the bridge is a great spot but not necessary). Once you get to the first zombie hit the jump button to vault over it, while in the air press and hold the attack button and as long as there is a zombie below you will blow it's head off regardless of how weak the wrench is.
. Crush head. Nothing happens. I have done this more than 50 times. Maybe it's glitched since the update?

First of all, make sure you got a wrench, also the two skills "vault" and "drop attack" unlocked. Next, go to Infamy Bridge (best spot because of the insane amount of zombies there) and just vault over a zombie and kill either the zombie you vaulted over or another zombie nearby. Obviously, you'll more than likely get this achievement while playing the game normally but I made a guide of it anyways. I'll post a video guide of it as well, from Powerpyx.
Edit: I forgot to mention you have to press LB once you've pressed RB to jump over a zombie, to actually do the execution move on a zombie nearby. Best way is to line 2 zombies so that you plan which zombie you'll take out.

I'm assuming this will probably always be there.
Also, I had to do this achievement 12-15 times before it popped, not sure if there was an interruption in services or what the deal was but just when I was ready to give up I tried one more time (probably an hour after first trying) and it popped. Interestingly enough when it did popped the time/date actually preceded the two achievements I had earned later on.

Just let him out and listen to his little enthusiastic speech then let poor guy fight his way to his gruesome death while you stand back and watch. The wrench should be close to his corpse.
Pick it up and run towards a group of zombies hold
as you approach a zombie to vault over it, hold 
while in the air to smash it down on another zombie for a one hit kill. You'll need Vault from the agility tree (Level 4) and Drop Attack from the power tree (Level 10).

Run at the first zombie and press
to vault over them then you need to press and hold 
while you are in the air and if you come down on a zombie you should smash it's face in and unlock the trophy.Here's a video of me doing the move needed.

You'll find it approx. 40-50m South East of a safe house in the slums... the one to the furthest South East of the map to be exact.
Once you unlock the required moves in your skill trees simply look for a few zombies in a group, Vault over one and hit R1 to perform the Drop Attack. Mario!!

I recommend keeping a wrench handy, at least in your stash, so once you have leveled Agility to at least level 4 for Vault, and Power to at least level 10 for Drop Attack, you can grab the wrench and get this done immediately.
To perform this combo, you must be sprinting at an enemy, and then tap 
to vault over them and 
while you are over an enemy to drop down on top of them and crush their skull with your wrench. This is easily done on a group, vaulting over one enemy and crushing another, but it can be done on a single enemy if you vault and pull back on the 
slightly before pulling the trigger. The video below shows this being done. Full credit to its creator.

You need to be at least power level 10 and agility level 4 before you can attempt this trophy. Once you are the required level you want to purchase both the "Drop Attack" skill, which is in the power skill tree. You also want to purchase the "Vault" skill, which is in the agility skill tree. Once you have both you now need to equip a wrench. At lower levels you can buy them from vendors and you will find them as random loot more often from chests etc. However, once you are a high level then getting them to spawn can prove difficult. The best places we found to find one are:
- Taurus Gas Plant - You can find them here on zombies, if zombies have spawned in the gas plant itself. Most of them should be carrying a wrench. The plant can be found HERE.
- Train Track Warehouses - Sometimes, you can find zombies in the warehouses running alongside the train tracks. Zombies near them may be carrying a wrench.
- Antenna Map Tunnel - In order to reach the antenna zone, you need to go through a long tunnel. Most the zombies here are carrying some sort of weapon, and quite a few of them are carrying a wrench. The entrance to this tunnel post game can be found HERE
Any wrench will do, and there are quite a few types in the game. Just as long as it has "wrench" in the name it will work for the trophy. Now all you need to do is run into a group of zombies, vault over one with 
and then hold 
while in the air. Hopefully, the attack will hit a zombie and instantly kill them. The trophy should unlock shortly afterwards. Here is a visual walkthrough:
