You can get this achievement from the start by choosing either Liu Bei, Guan Yu or Zhang Fei during the Yellow Turban Rebellion scenario, they will become sworn siblings upon starting.
Alternatively you can do this with any character by levelling up your relationship to S rank with another person and then becoming sworn siblings with them whilst on a stroll, the quickest way to level up relationship is to interact with someone whilst on a stroll.
If you are compatible with them then you can get up to 20 relationship points per interaction which means it would take 5 turns or less to reach the 100 points required to reach S rank (compatibility is based upon the traits you level up, so if you are level 10 evil then you would get 10 bonus points when interacting with Dong Zhuo for example).
