I made a video scrolling through the entire list of Stories & Records:
In my case, even after finishing all six endings and doing everything I could find, I still had a few Story cutscenes/dialogue and some Music tracks missing. And it was hard to figure out exactly what I was missing since everything you're missing is labeled "???", lol. So I hope this helps. I'll add notes here about the easily missable ones when I have time; I at least wanted to put this video up for now to help those who are almost done and still trying to find the last few "???" entries they have

**Missable**
You will unlock the Living Well of Knowledge achievement as part of unlocking Spirit of Camaraderie (30G). This is because it requires you to have uncovered every letter, every loyalty path, and every story. Together with the manual save that we make in Chapter 3, this allows us to go down each of the three loyalty routes and to uncover the content exclusive to each of the routes. By the time you get to the end of the game and go into New Game +, this achievement will then unlock.
