At a certain point in the story, you must choose a faction you want to side with; the game will make it very clear when you have to make a decision and are passing the point of no return. The faction you choose, and how you play specific mission(s) later on will determine which of the six different endings you will receive. Note that while this achievement is for one ending, and there are five others to get, you DO NOT need to make a new game to get the rest of the endings! Once you complete the entire story once, you get the option to go back and replay story missions, and even replay the game from any chapters, while keeping all the weapons, skills and any other unlocks you've earned (it's essentially New Game+). This means you'll be able to go back and get all six endings, all on the same save file, so don't worry about that.

How to complete the Wu story with the TRUE ending (story spoilers, including story characters that die and how you must save them):

You must side with the Sun clan in Chapter 3. HOWEVER, in order to get the Wu story's TRUE ending (NOT the normal Wu story ending, which is a different achievement) you have to save two important characters that would normally die at scripted points in the story (you'll see this in the normal ending path of the Wu story). Both of these characters will require fighting tough boss fights to save, but the first one has such a tanky boss and tight time window that it's basically impossible on your first playthrough.

So, if you haven't already, I would recommend playing through the story at least once a different way so you can enjoy the story without spoiling who dies! Afterwards, you can replay the story from the exact mission(s) where you need to save these people, so you can get this achievement later when your character is stronger (remember: when you replay story missions later on you keep all your weapons, skills, unlocks and etc. up until that point). If you're ready to get the true Wu ending then please read ahead:

FATE ALTER #1 of 2: Save Sun Jian in the Chapter 3 (Sun) mission Battle of Xiangyang. Here is a map of the path and steps I recommend taking to do this:

It's pretty straightforward, but the boss (step 5) is super tanky and is difficult, if not impossible to beat without playing through the game at least once.

If you are SUCCESSFUL: you will see two cinematics; one where Sun Jian dies, and then another showing he was saved. You will see eventually see the text "Fate Altered" and the mission will end.

If you are TOO SLOW: you will only see one cinematic. Sun Jian will die and the mission will continue, with the objective for Sun Ce to retreat from battle. If this happens, you must die or withdraw (pause the game and go to System -> Withdraw), restart from an earlier checkpoint, and try to get to Sun Jian in a faster time. Remember, you can always lower the difficulty to Historian to make fights easier; it won't affect achievements.

After saving Sun Jian successfully: if it's your first playthrough (how? literally how? I want your autograph) simply continue the story as normal until Chapter 4. If you've already completed the story at least once before, then you can now skip to "Play from Suppression of Wu" near the end of Chapter 4 (Sun).

FATE ALTER #2 of 2: Save Sun Ce in the Chapter 4 (Sun) (true ending path) mission Battle of Xiangyang. Here is a map of the path and steps I recommend taking to do this:

As far as I know, you don't have any kind of rigid time window to alter fate in this mission, unlike the one to save Sun Jian. However, if Sun Quan is ignored then he can easily die from the many officers ambushing him, so it's important to start out by helping him before running off towards the boss. Again, for more details please see the map I linked to above.

If you are SUCCESSFUL: Bailuan will flee and after some dialogue you will see the text "Fate Altered". Then you are free to leave the cave and finish the rest of the mission as normal

After saving Sun Ce successfully: Chapter 5 starts immediately. If you've now saved both Sun Jian and Sun Ce, and you've already completed the story at least once before, then you can now skip to "Play from Suppression of Wu" near the end of Chapter 5 (Sun). You can play the remaining two missions as normal (they should be virtually identical to the missions as they were in the regular Wu story) and this achievement will unlock at the end credits

In order to unlock the true ending of the Wu story, you need to complete a couple of optional objectives: first, save Sun Jian at the Battle of Xiangyang, and, second, save Sun Ce at the Suppression of Wu. You can save Sun Jian by ignoring the intruction to push on and leave him and heading to the large central area of the battlefield. Here, head to the cave at the very bottom-left corner of the map, and you will find Sun Jian.

For Sun Ce, during the Suppression of Wu, wait until Sun Ce captures a base near the centre of the map. Then, go to the bottom left-hand corner of the map. Here, you need to beat phantom Yan Baihu; then head north and into the cave, behind the vines on the left-most wall. Here you will face and defeat Bailuan. After this, you will get the “Fate Altered” message popping up again. Complete the rest of the battle.

Then, proceed through the story and complete the Battle of Sanjiangkou and the Battle of Chibi as usual. When you do, the The True Future of Wu achievement will then unlock.
