You unlock the ability to reforge the traits of a weapon after completing the story of one of the three factions: Shu, Wu or Wei.
You have to go to whatever city and underneath weapons shop there will be an option called Reforge. Click on it and you will be able to move traits over from one weapon to another that are the same grade of weapon. The grade is the number in the bottom left of the weapon and usually has the same name as well.
Just pick a weapon and move over one trait to another weapon for 1000 currency, unlocking this achievement.

Once you finish the game you will be able to reforge weapons, this is available at the weapons shop and requires you to replace a trait on a weapon with the desired one from a different weapon. This destroys the weapon you took the trait from. You can only use weapons that are the same.

When you complete any one of the three pathways that open to you in Chapter 3 – Shu, Wei, and Wu – you will unlock the abillity to reforge the traits of your weapons. In order to do this, you need to go to any one of the cities and head to the weapon shop there. You will see an option there to “Reforge,” and that’s what we want to do. This lets you move traits over from one weapon to another, if both weapons are in the same grade, of course (that’s the number in the bottom-left corner of under the weapon).
It costs some currency to do this, but once you have moved that trait over, the Weaponsmith achievement will then unlock.
