This isn't quite as simple as just driving head first into an obstacle to damage your engine.
First off go Quick Play >Solo > Create Rally > Create Championship > Add Event > then choose your preferred class of car > Vodafone Rally de Portugal > choose your preferred season > now add multiple stages (I filled it with the max 24 stages just to be safe and used varying lengths of stage).
Keep in mind you'll be driving a damaged car so I would advise not to choose the long 30km stages. I personally completed this after 3 stages all around 15km but can likely be done using shorter stages. With each stage added you'll want to have a Long Service Area selected for each so you can repair any other damaged components.
Now once you have the stages set up Confirm Event > Confirm Championship > Championship Settings > AI Performance to 1 and Hardcore Damage set to ON > Start Championship
After selecting your car and starting the rally your going to want to drive aggressively and damage your car enough that your radiator has heavy damage. (Top right icon on the speedometer will be red). Once the radiator is damaged just drive the stage normally and try not to damage your car more. Your engine will overheat and become damaged in time. You'll know the engine has heavy damage once the bottom right engine icon is red.
If your engine doesn't have heavy damage after the first stage just continue the rally normally, repairing any component other than the engine and radiator. Once the engine is showing heavy damage you can repair the radiator, and continue the rally winning the next stage.

Set up a custom championship in Portugal using 5 or 6 stages (7-8km each). Make sure to add a long service area for each one so that you can fix any damage unrelated to engine or radiator. Hardcore damage and very easy difficulty are also very handy so feel free to activate that as well.
The first thing you want to do after starting the first stage is to find a nice rock or a wall and bonk your car head first. Hit it at around 50-60kph, any faster and you risk getting an instant DNF. Also make sure you hit the thing as straight as possible to avoid suspension damage or a puncture. After a solid hit your radiator temperature indicator (next to the speedometer, lower right corner of the screen) lights up red, you're all set and you can finish the stage normally, but ideally without hitting anything else. If you set up your championship AI difficulty to 1 (lowest) you can easily win every stage without changing gears. Just redline it on 1st and let the engine cook. The achievement should pop around 4th or 5th stage. Once your engine stars cutting off power that means you're close to heavy damage. In my case it happened on the 5th one, but funnily enough I started it with medium damage and the red engine indicator appeared mid stage. It still counted for the achievement.
