This achievement automatically unlocks as part of the story.
Once you have finished a tile-based puzzle on the floor most of the way through a winding path with Dark Mirror enemies, you'll come upon a path leading to a pool of water in the distance. This pool represents the no-turning-back point. Use the save statue to prepare yourself and dive in. After a short cinematic, the achievement will unlock.

This is the final dungeon of the game, Stasis Chamber. Be sure to save your game so you can backtrack and finish any other unfinished trophies you have yet to earn. If you're ready to proceed, walk up to the big statue in the center of the room to trigger a cutscene. There's no turning back now, you'll enter the pool of water to the other side. You'll find yourself in the Memory Chamber as your party watches Ichabo absorbing Maáto's amri and the trophy will unlock.
