Earthroot Tea -> 2 Roots & 1 Inky Cap

Lyndowian Brew -> 2 White Bloomsacs & 1 Roots

Mountainwort Tea -> 3 Mountainworts

Mystery Blend -> Mix anything that isn't a recipe already

Netherleaf Tea -> 1 Mountainwort, 1 Scragweed & 1 White Bloomsac

Scragweed Tea -> 3 Scragweeds

Sesha Tea -> 2 Heavy Reeds & 1 Roots

I did this at Ranya's House, near the Great Shade and Blushwood Forest. She has a stove and kettle you can use to brew tea.

As Dale Baldwin mentioned in the comments, the Hydratistle is not required for this achievement. If you want to brew that one anyway, be careful as it requires 2 Black Thistles (and 1 Inky Cap), which will void the Keeper of the Forest achievement.
