Just before the end of the third level you will come across an octopus. You HAVE to go by him, so he's easy to find. Basically just go REALLY slow by him and he won't hit you. You have to go the opposite direction past him on the way back so make sure he doesn't hit you then either, and you unlock the achievement at the end of the level.

At the end of the Undercaves there is a large octopus waving a tenticle, swim very slowly and straight past him both ways and he will not hit you. Finish the level and you will unlock the achievement.
