Make sure you make a manual save prior to unlocking this trophy, so you can reload it and work towards the game-spanning loathing mini games.

The old man is located in The Collection room at the convent. You will need to gather many items in the convent before you are able to access this minigame – specifically: Three food items in the cafeteria, three masks in the stairwell and everything in the locked case in the classroom. Once you have everything, talk to the old man about his experience with Templars and he will give you several clues which will finally allow you to access this minigame.

It's a quick logic puzzle but the ultimate answer as to what to put on the shelf is: Angry mask / deer / coconut.
