Craft a 5-star accessory in the Accessory Shop on Second Street. I'm not sure the exact time 5-star accessories become available to craft, but it's probably late game, maybe even after a post-game quest. Just keep completing Squash's quests and eventually the Accessory Shop will be upgraded enough for you to craft them.

After completing the story you can craft level 5 Accessories. Save your Lapis Lazuli, you'll need them!!

You will need to create a five-star accessory for this achievement on Second Street. Again, complete side-quests to increase the stock. You'll need one of those precious gems I mentioned in the Road Map to do this, so if you don't actually want to keep the accessory you can save prior and reload so you don't waste a gem on this and can instead keep it for the armor/weapons.
