If you're familiar with Mario Kart, then this item is basically the golden mushroom.
The sandwich is an item that you'll get, most likely in last place. When you get it, just mash X as fast as you can until the powerup is gone. Don't even worry about crashing into a wall, just stay on the track and mash X.

Once you collect the sandwich from a pick-up barrel simply spam the 
button as fast as you can for little speed boosts. Do this 27 times with one sandwich. This is 27 presses of with the same sandwich, so make sure you press it fast!
