How to unlock the Great Rune achievement in ELDEN RING - Definitive Guide
To do this, you must first beat your first demigod boss; Godrick The Grafted at the Stormveil Castle. Upon defeating him, you'll earn his great rune item, shown in your inventory as a Key Item.
Now, you must reach the Limgrave Tower Bridge; which is easily accessible from the Liftside Chamber Site of Grace. Once you reach the bridge, go across it, defeat (or skip) the 3 large enemies, and find a teleporter at the end of the bridge. Use it to get to the Divine Tower, and then climb the stairs to restore the rune and earn the achievement.
Don't forget to visit a Site of Grace to equip this Great Rune after restoring it. And don't forget to consume a Rune Arc to grant yourself the effects. This one grants +5 on all attributes. You must consume a new Rune Arc upon death to grant yourself the effect again.

What exactly is glitched then? According to the game it's not supposed to...?

If you take everything slow and forgot about powering the first Great Rune (like I have...), you'll get this naturally through story progression.
I recieved this automatically after defeating the second demigod boss: Rennala, Queen of the Full Moon.
The Great Rune comes already powered. What it does is gives you the ability to respec your stats with
proving you have a Larval Tear, of which there are many mostly spread around mid to late game. Be wary of spoilers if you look them up.

It is not sufficient to defeat a boss with a rune; you must activate it before you can use it.
In order to do so, you must go to the top of the appropriate great tower; the rune's description will tell you which one. These towers are usually not straightforward to enter, either requiring a mini-dungeon or some trickery to get into.
Godrick's Great Tower - likely the first one you'll have access to - can be entered after defeating Godrick. All you need to go is to go backwards through the castle's main area towards the main gate - this is the route you take into the castle if you tell the servant to open the main gate, but in reverse.
There, you will find a wide-open area with a exit to the east. This leads to the great tower.
This is visible on the map, so isn't terribly difficult to find. Note, however, that you will have to defeat several mini-bosses to get there - a lion with a weapon, plus a few golems - though there is a grace point between them and none of them are especially difficult.

There are a total of 7 Great Runes, 6 of them are dropped by the Shardbearer Bosses while the other is dropped by Rennala. After getting any of the Shardbearer's Great Runes you will need to go to a divine tower to activate it while the one you get for defeating Rennala is already activated and the trophy will pop along with the one for defeating her if you have not activated another Great Rune before then.
- Godrick's Great Rune - Activated at the top of the Divine Tower of Limgrave.
- Great Rune of the Unborn - Automatically activated when obtained.
- Radahn's Great Rune - Activated at the top of the Divine Tower of Caelid.
- Morgott's Great Rune - Activated at the top of the Divine Tower of East Altus.
- Rykard's Great Rune - Activated at the top of the Divine Tower of West Altus.
- Malenia's Great Rune - Activated at the top of the Isolated Divine Tower.
- Mohg's Great Rune - Activated at the top of the Divine Tower of East Altus.

Can be done immediately after killing Godric. In the castle courtyard (which is immediately after the main gate) we turn east (the passage is guarded by the Witch Lion). There will be a long bridge with three giants, you can simply run over them - at the end of the bridge there is a teleport to the tower. Get up, pick up the rune and get the trophy)

Godric's rune is the tower on the right at the entrance to the castle, if you go through the gate. The guard car is also a dog with blades - run through all this because there is grace right at the entrance - then along the bridge to the teleport
the Radana rune is a tower in the north of the map, next to it there is grace with a merchant. go from there. You can get there by following the roots of the trees. Also, when climbing up, you will need a horse because you will have to jump over the cliffs on the tower
Renala's rune - no need to illuminate - works immediately - gives rebirth - and its tower is in the east of the region, you can get there with the quest Clock
Moga rune - in the western tower of the capital - access when leaving the capital in the east, when you go to the winter area
rune of Malenia - access to the tower is possible before the burning of the capital. You need a teleport in the capital, which leads to a secret tower outside the capital. In order not to reach the building which is a clone of the round table, there will be a teleport at the top
Ricard's rune - in the eastern tower of the capital - access to it through the shaft
rune of Morgoth - in the western tower of the capital - access when leaving the capital in the east, when you go to the winter area
