Still didn't get this achievement
I beated him very early in game and still dont pop up when finishing the game
Also the regal ancestor spirit achievement dont pop with me....
I only need these two achievements for 1000 score.....

I can't seem to find this location after having burnt the erdtree. The capital is mostly under sand and a lot of the sites of grace shown in the video are gone for me. Does this mean I need to try on another playthrough?

Here's a written guide explaining the attacks Mohg, the Omen uses, and how to deal with them. Recommended you be around level 100. And bleed spammers...he's immune :p

--==Physical Attacks==--
Note: Most of Mohg's physical attacks have delays to them. This will challenge your instincts to roll too early, but hopefully a hundred deaths to Margit taught you something.

Thrust
A simple thrust attack from two-handing his trident. This attack is very easy to punish with a guard counter. If instead rolling, always to the side.

Overhead Slam
He will raise his trident high behind him and slam it down forward. The range is long, so roll to the side. The worst of the delayed attacks. Watch for the weapon coming down, not his actual arm.

Swing
Rare to happen without some form of combo, Mohg will swing his trident in a horizontal arc. You can roll under it but if you are close enough to him, it usually goes right over you should you just circle around him to the right. If needed, you should favor rolling over blocking in case it ends as a stab combo:

Trident Stab Combo
If Mohg had started with a swing without casting magic, there's a good chance this will be a combo ending with him stabbing his trident into the ground in front of him. After a delay he pulls it out, and you can take damage from that so don't stay by it. The final stab attack does a ridiculous amount of stamina damage should you block it which can leave you staggered, almost guaranteeing you take the follow-up damage. If you are near him, circling around him to the right can avoid the combo but feel free to roll and keep the shield up in case. He usually swings three times before the stab, but can do it after only one swing too.

--==Blood-Flame Magic Attacks==--
Note: All his magic attacks cause bleed buildup and continue to build after leaving contact shortly after.

Spray (and pray)
Mohg will reach above him pulling blood magic out of thin air, and then throw it at you. It can either be in a horizontal arc or straight line. I found it best to roll towards him diagonally to the right as you'll usually avoid this regardless of which way it appears. Getting hit by it though isn't as bad as standing in the flames it will leave on the ground after. He usually does this attack when you have distance from him.

It's Raining Men('s blood)
This attack has the same reach above animation for pulling out blood, however instead of throwing it at you he will drag it to the ground causing blood to rain around him for some time. The radius covers him in all directions but will be widest in the front. The rain leaves flames on the ground that do a surprisingly large amount of damage should you stand in it. Therefore, don't get greedy and poke him during this attack if you want to play it safe and simply run away. This is your best time to heal if needed. He usually does this attack while you are near him. Note, you can attack during this if you want, I just don't recommend it unless you're a fast build.

Claw Explosion
Mohg scratches claw marks into the air in front of him. Shortly after it explodes. The warning is quite obvious but the time to react is short. Therefore as soon as you notice the clawing, circle to the right around him and roll if needed and the explosion should miss. You can roll back as well but in my experience this was less effective and blocking was better if I couldn't get around in time.

Combustion Swing
I call it combustion because it has the same animation as the combustion spell in past Fromsoft titles. If you are unfamiliar with it, he will charge up his hand at his side and throw it forward creating a larger explosion in front of him. There is additional warning as before this he places his trident's staff end onto the ground. He will always swing his staff after this explosion. Like the smaller claw explosion, getting around him to the right will let you avoid this. Since there's a little more warning this time though making distance quickly can also be effective.

--==Final Notes and Additional Tips==--
  • Mohg can be staggered if you do enough damage. When you hear the sound effect and he kneels down, get close in front of him for your critical attack.
  • At the bottom of the sewers past some lobsters (will need to jump down from pipes section with a bunch of imps) you can find Mohg's shackle. Similar to Margits shackle this will stun Mohg on use for some time. Can only be used twice per attempt.
  • If you have done the Dung Eater's quest to the point of freeing him from his cell, he can be summoned to this fight.
  • For melee players, many of his attacks can be avoided by circling him to the right if you stay close. Create distance from him when you need to heal though.
  • Your biggest threat of blood loss is from standing in the fire more so than getting hit by attacks, but even so the "Stalwart Horn Charm" tailsman (increases resistance) and the incantation "Bestial Constitution" or crafting item "Stanching Boluses" (for removing buildup) can all be useful items for this fight.
  • After defeating him, be sure to hit the wall behind the treasure chest ;)

As part of the main questline, you will get there almost at end game, he is going to be at the bottom of the sewer in the capital, thats the mohg that give the achievement.

Location: Leyndell - Subterrenean Shunning Grounds
Suggested Level Range: 70-80
Damage Types: Standard, Pierce, Fire, Hemmorage
Weakest To: Standard, Slash, Strike, Pierce
Drops: 100,000 Runes, Bloodflame Talons

If you have Mohg's Shackle, equip it as it will allow you to stun Mohg, the Omen. Wait for him to approach and use the shackle to stun him and wail into him with some heavy attacks. Depending on the weapon, you may want to trade a blow or two to break his poise so you can get in a critical hit. Afterwards back up and heal, plus rebuff if you need to. Now you are going to want to read and avoid the attacks. Mohg will leave trails of Bloodflame on the floor and splash out Bloodflame that will deal fire damage and build up toward hemmorage. Roll through his attacks, watch out for his Bloodflame Talon as it will not do a lot of damage but has an explosion that deals high damage.

If you are looking for it after burning the tree, then the path to it will be through a large hole in the ground in the center of the city, not far from the place of grace to which you will be teleported.

after the boss there is a chest, behind it there is an altar, if you hit it it will lead to another location
