After reaching the Forge of The Giants Lost Grace, you can choose to Talk to Melina and tell her that you are ready to commit a cardinal sin. A cutscene will play and the achievement will pop.

I had to just go through the game again on NG+ to get it to unlock

Surprised no one did this until this point. Guys, beware:

This achievement happens after the Fire Giant fight in the Mountaintop of the Giants region. It is a POINT OF NO RETURN in the game and may void some achievements.

If you're going for the cloud save & all endings part then I strongly advise you to make sure you have stuff taken care of before you light a fire.

I have not beaten the game yet and will update the solution with more info once I am there.

Story Related

After defeating the Fire Giant, move upwards to the large Kiln and head to the Site of Grace on the rim of it. At the Grace you will have an option to Speak with Melina, select it and a cutscene will play.

Behind the Fire Giant, you can follow the large chain to reach the Giant's Forge bliss (go through the left wall of the forge to reach it). Rest in this blessed place, this will give you the opportunity to talk with Melina. Talk to her and she will set the tree on fire (unlocks story trophy). This will also take you to the final region of Collapsing Farum Azula.

Also make sure you pick up the missing legendary weapon Bolt of Gransax in the Royal Capital (it disappears after defeating Maliket).
