[Disclaimer] This is my first time posting here, so bear with me. I'm writing this up because this fight was frustrating and took over 8 hours playing around with multiple solutions, equipment, and specs to get it right for my play style. I hope this helps someone.
Getting to Malenia
Malenia can be found at the bottom of Ephael. To unlock the zone, you first need to progress the story to the point where you have access to and defeated Commander Naill in Castle Sol, and unlocked the western part of the Mountaintop of the Giants.
Once there, head to the northern most section of the map and you will find a site of grace, Ordina, just outside a ruined town north of the frozen river. You will need to use the Evergaol located in the northern part of the ruins. In the Evergaol, you need to light 4 braziers. They can all be found outside the Evergaol, so you can map out your route safely. Each lit brazier will remain lit, so you can focus on the missing ones. I found that running while on the ground kept me from dying to the assassins. Using a shield flask helped get to the ladder next to the magic archer on the rooftops (the hardest to reach). After that, head to the newly opened gate at the northern edge of the town and proceed to the new zones.
Work your way through the Haligtree, defeating Loretta, and navigate through Ephael.
Fighting Malenia Phase 1
Malenia is fast. She heals every time she hits something with her sword, even if it is blocked. Bleed and Frost appear to work best on her, or anything that can knock her down. If you get her backed up against the wall in this phase, you can stun lock her and keep her from attacking. When she jumps into the air, put up your shield, run, or get ready to dodge. She will unleash a flurry of attacks in 3 sets with a small knock-back at the end.
Fighting Malenia Phase 2
Phase 2 has all the mechanics of phase 1, but adds a few new twists. She will start with her big opening move, jumping in the air and hurling herself at your location. After landing, she looks like a flower that is pure rot and will explode shortly after impact. The impact, explosion, and rot will all do massive damage or one-shot you. Shielding through the move while locked on an backing away is the easiest method to survive. You will get knocked back from the explosion, but should live. She has another new move that also spreads rot near her. In general, when she goes in the air, move back a ways and put up your shield just in case.
Methods that Worked
I found the following methods worked best for me and a friend.
A Faith build with the Blasphemous Blade +9 (from the old woman next to the two fingers at the roundtable) worked best for me. I spammed the special attack (LT) while using a shield that did not have an ash (Brass Shield in my build). The Mimic Tear +10 was the best summon since they will also use the special attack. The sword's attack unleashes a cone of fire on the ground in front of you, knocking down Malenia on the first or second hit. Timing is key, so alternate with your mimic and have time to flask if you need more FP. If she is in the air or dodges (frequently a problem), the attack will miss.
My friend used his Intelligence/Strength build with the Hoarfrost Stomp ash of war (his go-to close attack), a shield for blocking, and Black Knife Tiche +10 for the summon. The combination of spamming the frost stomp with Tiche's speed and bleed attacks did the trick. He also tried with the Mimic Tear +10, but had more success with Tiche.
I had some luck with bleed applied to dual wielding katanas (both +9) in a Dexterity build and the Mimic Tear +10. I also tried bleed on the main hand and frost on the off-hand. Unfortunately, I couldn't consistently dodge and survive the big attacks that I mentioned shielding through earlier. I even tried summoning my Mimic while wielding both swords, then swapping the off-hand for a shield. This ended up being a pain to remember doing repeatedly and I couldn't attack fast enough to drain her health. The first and second methods both have the advantage of covering a large area with distance.
Bear in mind that none of these options are easy wins. You still need to learn the fight and recognize the mechanics to survive. Hopefully those tips are clear and help someone in the community save some time and frustration. Thanks.

This is a video of how I cheesed this boss as the Samurai Class. Weapon Used Uchigatana+25 with Keen Ashes of War Spinning Slash and mimic tear ashes, this works in both first and second phase. Hope this can help others who have struggled.

Took a change of gear and a handful of tries, but boy howdy did it work. Only thing I had different was that my katana was cold instead of keen. Extra frostbite procs with the bleeds really chunked away the health bar.

I've beaten Malenia as a caster without changing too much of how I played the rest of the game. I just wanted to share the way I managed to do this boss because I had a lot of trouble finding something that fitted my playstyle.
The key element to beating this boss in almost any video is: be aggressive and barrage her with attacks so she gets interrupted / staggered. The other element as might be obvious in any boss fight, but especially this one, is try to avoid getting hit at all costs because she'll heal herself by damaging you.
Sorcery
To achieve the barrage element, I went through the list of sorceries and tried several. I eventually settled to do it with just Rock Sling because it's pretty fast, knocks her back a bit, it locks on well enough most of the time and it is a pure intel sorcery (unlike ancient death rancor).
To help with the barrage you'll use your mimic+10 casting the same as you. Keep in mind that mimics are stupid so if they have other sorceries or a melee weapon they'll do other things than you want. So I suggest to memorize the one spell you want to use. I also refrained from using the aggro talisman on this boss because the mimic was dying way too fast in phase 1.
Try to position yourself so she's in between you and your mimic, I found that that's the best position to make sure the rocks hit her.
Gear
- Dual wield staves: Lusat's Glintstone Staff +10 / meteorite staff (for gravity boost)
- Queen's Crescent Crown (for the +3 int, to get to exactly 80int) and whatever gives you the highest physical damage negation without fat rolling.
- Graven-School, Graven-Mass, Radagon Icon and Crimson Amber +2 talismans (for survivability)
- Flask of Wondrous Physick had Opaline Hardtear (shield) / Cerulean Hidden Tear (temporary no fp cost)
Disclaimer
I have to say this is by no means a guaranteed kill as she sometimes just dodges almost everything or slaughters your mimic early on. She also hits like a truck so even with loads of health she can bring you down really fast, so learning her attacks and learning how to dodge them is vital.
For the people dying a lot at the start of phase 2, run towards her and once you're under her dodge. This has prevented me from taking damage most of the times.

This guide is assuming you already found Malenia and looking for a guide to beat her. If you are still looking for her location YouTube guide to her location or Haligree Roots site of grace (site of grace in front of boss door.)
I used this guide mainly but made a few adjustments. The person in the video seems to be safe by the entrance, but it didn’t work for me plus you can easily get cornered. So when she starts attacking your mimic run to open space.
Right hand: (1) River of Blood+10, (2) Fallingstar Beast Jaw+10; Left Hand: (1) Brass Shield+25 (has 100 physical damage block); I wore Radahn’s armor but as long as it’s something good it really up to you; Talisman: (1) Erdree’s Favor+2, (2) Starscourage Heirloom, (3) Winged Sword Insignia, (4) Shabriri’s Woe (switch to Greatshield Talisman after summoning Mimic). Level up your mimic as much as possible, my was +9.
Equip River of Blood sword and when you enter summon your mimic right away and change your 4th Talisman. The goal is to have your mimic attract her aggression on him and to give yourself the best shield as possible if she comes after you. Keep your distance (but not to far) and equip Fallingstar Beast Jaw and spam (Xbox) or L2 (PlayStation), or whatever key on PC for special attack.
