saw some people in forums say they got stuck with caja not moving from the berserker fort.
here is what I did.
I did all the side quests in chapter 1, ignored the main missions. what you need to do is all the caja missions in chapter 1 that you can. stay on her good side raising her "meter". there is no actual way to check where you are in standing with her as far as progression, you can just check what she is currently at by asking her.
doing a "good" playthrough with LOW destruction, keep her in your party so you gain points with her when you make "good" decisions. I didn't do this and panicked, because I already finished almost all side quests. you get a lot of points with her by choosing "good options" during her companion quests. always support her and express interest in rekindling your relationship as well. by just doing this and a what very few side missions I had left, and eventually taking her on main missions (more on that in a min) always chosen good options, I was in high enough standing with her when the time came, it was either "true friend" or the one after that I forgot.

I did the usual sleep to midnight then morning to pass the days, which usually is a easy way to trigger the next companion missions. but nothing happened. so I progressed thru act 1 into act 2, and had to talk to caja about the main plot, and was able to also continue her companion quest! kept going doing her quests until it stalled and went to act 3 finished her quest line here and this quest popped up immediately after, affirm you want to try your relationship again and the achievement popped!

I finished the quest, talked to her after, chose the spend time together option. Still hasn't popped. Reloaded and did it all again, still nothing. Anyone else encountered this bug and have tips?

not sure if this will be any help. I would recommend you do not save over that save file! just in case.
you probably know this, but the xbox one and series have always had achievement issues, like delayed unlocks. sometimes achievements unlock days later or while your not even online. sometimes the achievement unlocks but never "pops" on screen. etc. etc.
make sure your connected to the internet as well.
aside from keeping that save file from before finishing her quest, and trying again. im not sure what tips to give you. maybe if possible for you, delete all the other saves besides that one, clear your cache, try signing out and back on your xbox. the usual troubleshooting stuff.
