How to unlock the Sewer Rat achievement in Elex - Definitive Guide
This achievement is related to the thieves guild "The Claws" located in the Domed City. It is kind of missable if you are not thorough enough. (Update: I checked again - you never have to go there)
There are two ways to enter the guild's hideout but first you need the entry token from Katta (Berserker). She is located on an island in south Edan and after you do a quest for her, she'll give you "Katta's Claw".
Note: Andrinho7x pointed out that you can get a claw from Rat (Outlaw) as well. He'll give it to you at the end of his quest line.
How to enter the hideout:
a) With the key from barkeeper Vito in Domed City. You can either pickpocket the key from him
You can get it from him through dialog as well if you have the claw in your inventory. After that you'll get the quest "Strange mechanism" which will show you the way to the trapdoor.
The trapdoor is in the west part of the city. To find the keyhole next to a bonfire, stand in front of the trapdoor and look up and slightly to the right.
b) Access the hideout through a passage above the teleporter ("Small Farm" or similar) south of the city. The actual entrance is north east of the teleporter. It is right above the small "shed" behind the house. You'll need the jetpack to get up there. Or follow the city wall and jump down.
Show the claw to bouncer Darius and then exhaust the dialog with Hector. Achievement should pop then.

If this achievement is missable, why isn't it flagged on TA as such?

This achievement is an optional side-quest related to joining what amounts to this game's version of the Thieves' Guild. There are a number of ways to get into the crew. Refer to A Strange Mechanism for how to complete the quest and unlock the achievement. Once you're in, a series of optional thieving quests open up for you.
NOTE: As I mentioned in the Road Map, make sure when you first arrive at The Fort, be sure to give Rat some money. Upon completing his side-quest chain Tramp Confederacy, he will give you an item called Rat's Claw which will be helpful during this quest chain.

The achievement is awarded upon joining the Thieves Guild near Abessa. You can get to them either through a quest, or by stealing a strange key from the bartender and going down into the sewer (the passage is in the center of the city, to open it you need to insert the key into the opening in the wall nearby).

When I was about to do a quest with them, the fate of the Dome City was decided, the city was destroyed and captured by the Albs. When I found a way to fly to them along the rocks, they were not happy with me at all, which made me realize that it was not worth finding them before the quest.
I managed to get through to them only when I was able to get to the bartender’s corpse and take his key. It took a long time to fight our way, killing a lot of Albs.
