You can force a 2 flame rating by getting the minimum number of clients to the rescue zone, then pressing start to bring up the menu and abandoning the job.

You can do this on any rescue mission including the first one, called First Ave.

Achievement Walkthrough:

From what I can tell, you get three flames for doing the bare minimum, at least for a rescue mission. So in order to get a two flame rating, you will need to the bare minimum and be seen stealing to be docked a flame. I found Dent Memorial Bypass to be a good one if you drop clients under the basket elevator. They will eventually stand back up and have direct line of sight to your truck, almost guaranteeing they spot you stealing.
