How to unlock the Bulletproof achievement in Empire of Sin - Definitive Guide
This is actually easier than it sounds. If you lose a crew member at anytime or happen to have a whole party wipe just quit and reload. I have had a total party wipe with game over screen and just hard quit to desktop and relaunched game from earlier save with no issues.
I’m not sure if reloading from the game over screen actually counts, but why take the risk? Hard quit and reload!

Just wanted to add this for peace of mind. Fighting a battle with your own guards or an ally's guards will NOT void the achievement if they die. Same with Lone Wolf. It is strictly for recruited gangsters that impact the achievments

I just beat the game and no one in my sqaud was killed. However, Hugh "Tank" Miller just dropped dead on me in 1921. I reloaded and he kept dropping dead out of combat. I beat the game and no achievement.

To add onto the other solution, ONLY gangsters from the black book matter.
If they were once part of your crew and they die, it voids the achievement. For instance, if one of your gangsters defects to an enemy gang, and you kill him during a gang war, it voids the achievement.
