Loyalty is gained the longer a gangster is under your employment. So to get it to maximum, simply keep them alive and pay their salaries. You'll eventually get this though it may take a bit
There are other ways to increase it, such as making them into lieutenants but that also requires noteriety which is gained by killing people.
