Angelo is leader of the Genna Crime Family, so you'll need to target them for this. Him and his gang won't always spawn in with certain settings, so if you need or want this cheevo then reset if he doesn't spawn
You'll want to declare war on the Genna Crime Family and begin to take their rackets and kill their gang members. Do this until their safehouse is revealed. Before going in, I'd suggest saving as its going to be a hard fight and you might need to reload the save. Kill all Genna gangsters in there (killing only Angelo and fleeing doesn't count anymore) and then you should get it. Don't worry if you kill him and don't get it mid fight as it only unlocks AFTER the fight is done.
