How to unlock the Good Heart achievement in Eventide: Slavic Fable - Definitive Guide

After you complete the Slavic Fable story, you will unlock the Midsummer Night one. Just play through it completely and after you finish it off the trophy will pop.
The, Pants, Party
The bonus chapter unlocks after you complete the game once. There are no extra achievements tied to it, so you simply need to beat it using whatever method you'd like, hints and skips are fair game. The written walkthrough I used didn't have this chapter included, but the video below should help you if you get stuck.
