Travel west of Papunika Village, past the Crystal Caverns, and walk around the lake till you enter the hidden meadows. There is a hole in the ground in the area. Go in there and achievement will pop

Near the beginning of the game, before you enter the Crystal Cave. Head West of the cave opening to a dead end path and walk over the 4 trees there to find a hidden area. In that area there is a hole in the ground, once you enter the hole the trophy unlocks.
