How to unlock the Up The River achievement in F.E.A.R. 3 - Definitive Guide
This achievement is unlocked by finishing interval 1 with a score of at least 95,000. I found that the achievement is easier to do in solo campaign as coop players' scores are not added together at the end of the interval for these achievements and they must individually reach a score of 95,000. Here are some challenges that should make it easier to obtain these achievements. Aim to complete as many of these challenges as you can as well as any weapon kill challenges that are available in this interval. I got this achievement whilst playing as Point Man on Commando difficulty. The number after each challenge represents the total amount of points obtainable from completing that challenge as well as any challenges for doing lesser amounts of the same thing.
15 melee kills 3500
3 Special back melee kills 2000
5 Flashbang Blinds 2500
50 Headshot Kills 7500
25 Kills from Cover 9000
200 Seconds in Cover 2500
10 Kills After Mantle 4500
10 Kill After Slide 2500
60 Ammo Pickups 3500
50 Slo-Mo Kills 15000
3 Headshot Kills in 1 Slo-Mo 3000
5 Kills in 1 Slo-Mo 4500
120 Seconds in Slo-Mo 6000
Alma Doll 5000
Psychic Links 1500 each
You should also aim to get the bonus at the end of each interval for not dying or restarting from a checkpoint. This will also come with the challenge for killing 50 enemies in a row and on some intervals, 100 kills in a row.

Does difficulty effect the amount of Xp you get per challenge?

Im pretty sure there just a bonus amount of Xp for choosing a higher difficulty. The amount of Xp for each challenge should stay the same.

The following videos are not my own. They were uploaded by YouTube user savgcom. They helped me get this achievement on the first try by following his directions step-by-step. I hope they help you, too.
Using this guide I was also able to get the following achievement:
The achievement will unlock when you finish the level.
I got 119,250 points by following these videos. You need 95,000. That's a difference of 24,250, so you have some room for error. Good luck!

Thanks for the vids. Made most of it super easy. Took me hours to get through that one room after the 3 head shots in one slow mo without getting hit though, damn that was hard.

I hear ya! I think in basically all of the levels you can get by just fine without that extra 10,000 points, but if you're able to get it then it just makes your job that much easier. Honestly, about half way through the game I got the hang of it and I didn't really need the videos anymore. It'll get easier.

For each Interval/Score trophy you will need to maximize your points gain. This is easier without a partner as your individual score is needed to earn this trophy. In Interval 01 just practice getting headshots, using cover and using slow motion (or possession). You can earn four weapon challenges here (Strader, SMG, CAS, Frag/Flash) but there are not enough enemies for 100 kills. Be sure to grab the Alma doll and as many Psychic Links as possible.
