This is a story related achievement and cannot be missed.
This is related to finishing the quest "Try to stop Jack of Blades" and it focuses on you chasing Jack through Albion.
You will Spawn in Witchwood and your goal is to reach the focus site while dealing with infinite spawning enemies. You can either farm the minions for a while to gather a large supply of experience points, or you can just run past them and get straight to the focus site.
Once you reach the Focus site, Jack will teleport himself to the next focus site, and you will be teleported to Orchard Farm. Leave through the left gate to reach the Lake and you will run into Jack again.
Once again he will teleport away and you'll be moved to the Hobbe Cave. Run all the way through the cave till you reach the final chamber (You might of been here before when rescuing the young Boy for his grandmother). Jack will once again teleport himself away.
The final focus site is located at the Top of Headsman's Hill. You will spawn near the Bowerstone prison, so do what you've done for the previous 3 locations and move yourself up the hill towards the final focus site.
You will then be teleported to the Heroes Guild and you will need to speak to the Guildmaster before heading into the Chamber of Fate to fight with Jack.
Jack Boss Fight:
Jack shouldn't pose too much of a problem. He will try and keep himself afloat in the air while sending out magical projectiles and summoning Minions to deal with you. My personal strategy was to just spam "Multi Arrow" and wail on him with Skorm's Bow; however any ranged technique will work quite well. You should hopefully have a large supply of potions by this point in the game.
After beating Jack you will be given a choice. Regardless of your choice you will receive the achievement; however your choice will affect which Sword you will get in the future; don't worry however, they're both as powerful as one another so just make your choice based on your characters morality.
