This is a story-related achievement.
It is not obtainable simply by fishing and digging in the graveyard - you'll have to finish the main quest in the graveyard for this to unlock.
For the quest, one piece must be dug up, one piece must be fished out of a watering hole, one piece is in a coffin, and one piece is by the fireplace in the house. All of them have a green "mist" around them or their dig/fish spots.

Story related achievement, you will unlock this after getting the armor for Nostro. You will not get the achievement until you have actually finished the quest... you will get the spade and rod in respective chests in the graveyard.
