This achievement is for being either completely good or evil. I got this achievement after prison. It is pretty easy if you always make the same type of choices. If you want to do this earlier you can either donate to the Temple of Avo, in Witchwood, or sacrifice people to the Chapel of Skorm, in Darkwood. You can check your alignment by going to stats and hitting RT, you will get the achievement when you have 100 going either way.

Alignment boosts from armour can confuse the achievement, you need to strip down for it too pick it up right sometimes. (Thats what i had to do.
