This achievement is story line based. It unlocks after travelling from Hook Coast to The Lost Bay and the quest begins once you fast travel to Hook Coast.
A guard will tell you that the town has been evacuated and you need to place the fire heart on top of the lighthouse. Briar Rose will meet you in Hook Coast and the two of you will have to defend the lighthouse from Summoners until the Fire Heart is fully charged.
There are a total of three or four Summoners to kill and they will summon minions as well. You can ignore the minions and focus on the Summoners if you choose, once the last Summoner is dead the remaining minions will de-spawn. A life bar for the Fire Heart shows up underneath your map on the right side of the screen. After destroying the last Summoner a cut scene will occur and the achievement will unlock when you arrive in The Lost Bay.
