Edit: This achievement is glitched, so you have to open the Demon Dorr from the DLC "Traitors Keep" at first, otherwise the achievement will not pop up.
1. The demon door is on the prison island Ravenscar.
For this Demon door you must wear the prisoner outfit. Fot this you need: Bandana ; Top ; Trousers ; Shackles ; Boots
All this things can be found in chests on this island. For the trousers you mus complete the "Undead Sentence" to get this quest, just stay some time on the island. After finishing it you are near the chest with the trousers.
Wear all the clothes an go to the demon door, then it will open.
Credits for the last demon door goes to: ESS3NC3 0F 3V1L , thx for that.
2. Mistpeak Valley:
This Demon Door in the snowy area just up from the Lake. You you will need a Co-op partner. You can either marry your co-op partner in front of the door or go through all of the different positive expressions until it opens.
3. Brightwall:
Just inside the front gate of Brightwall and straight to the right. For this Demon Door you just need to bring your own child to it and perform a happy expression.
4. Sunset House
This Demon Door is at he sunset house whisch can be found in the wooded area to the left of the path after entering the region from Mourningwood. To open the door you must to be King or Queen of Albion.
5. Mourningwood
This Demon Door can be found next to the small village in Mourningwood. It requires the player to have a skill (melee or ranged) at level 5.
6. City of Aurora :
This Demon Door can be found near the trail that leads up to the city gate to Shifting Sands. You must to be 100% good or evil morality. If you fulfill this criteria, just walk up to it and it will open.
7. Millfields
This Demon Door can be found near the lake behind the houses that are below the small Millfields cemetery. For this Demon Door you are required to ignore fashion sense. You need to wear an outfit that makes you look like a fool (Women's Pyjamas or Men's Pyjamas work fine if you are of the opposite sex and if you colour the clothing articles different colours). This door also wants you to neglect your body so eat till you are fat.
If you are having trouble with it try sleeping in one of the houses near the door; it seems the door needs to reset before it will recognize the changes.

The counter when starting up the games read 6 of 7 for me. The DLC adds one door.

This solution is only for the newest demon door.
You need to wear the Prisoner Suit to open the last Demon Door. Here are the locations. Best to follow after completing every quest.
All starting locations are in front of the Keep entrance facing away.
Prisoners Shackles
1. Stand in front of the entrance to the Keep.
2. Go to the right down some stairs.
3. Go left and get through the hole in the wall.
4. Go right and vault of the rocks to reach the chest
Prisoners Boots
1. Stand in front of the entrance to the Keep.
2. Go straight and down some stairs and hills.
3. You will see some gallows, and a piece of the fence behind the gallows is broken.
4. Walk over to it, and open the chest to get Prisoners Boots.
Prisoners Trousers
1. Beat Faraday, and accept the Undeath Sentence quest.
2. Beat the last group of hollow men.
3. You will see the chest with Prisoners Trousers inside next to you.
Prisoners Top
1. Complete the main quests.
2. Go to the Keep entrance and enter.
3. Go to 2nd door on the left.
4. Check the 2nd cell on the left for the chest.
Prisoners Bandana

This is a helpful guide, I've been using it today.
Only, the description for both Bandana and Shackles state head "right" from the keep. I assume the description for Shackles should read left?
Also, you should think about wording it differently, like "While facing the keep, head ~direction~" this will avoid the confusion I felt when I got to the keep, read the description, and thought 'so which way is left, and which is right?"

Most of these are relatively straightforward after completing the majority of the game, assuming you have decided to stick to the extremes for choices (pure good or evil, good food or fatty food, etc.)
***Ravenscar Keep Door:
It is important that this is not the last door you unlock, unless you already opened every door prior to downloading the DLC. This achievement has been know to glitch for older save files
-->Wear every piece of the prisoner outfit in front of the door.
----->Prisoner Bandana: In a chest hidden in the small rock maze near the garden. To get to the garden, go down the left pathway when facing the bridge to the entrance of Ravenscar Keep.
----->Prisoner Top: In a chest located in the second cell on the left in the left wing of Ravenscar Keep (requires the quest Prison Management for Beginners to be in progress or complete).
----->Prisoner Shackles: In a chest which is accessed by vaulting off the rocks to the right of a hole in the wall of the garden area (the same hole which leads to the Demon Door).
----->Prisoner Trousers: In a chest at the end of the cemetery (requires the quest Undeath Sentence to be in progress or complete).

***Millfields Door:
-->1st, get as fat as possible. To do this quickly, go to any food/beverage store (Bowerstone Market, Bowerstone Old Quarter, Brightwood, Driftwood, City of Aurora, Mourningwood, Bowerstone Industrial/Understone) and buy everything the makes you gain weight (pies, mutton, fish, beer, wine, etc.), but hit X to eat it on the spot while buying, or eat during combat if time isn't a priority. While doing this myself, my weight bar was actually inverted for some reason, so just keep in mind that it needs to be on one of the extremes; if you've been eating fatty foods but your bar is green, this probably happened to you as well (it still works though).
-->2nd, become ugly. To do this, wear the pajamas of the opposite gender, the chicken gloves, and the mercenary tattoo set (all of which can be bought in Brightwood); the mohawk hairstyle also is considered ugly. Then, dye every piece of clothing a different color. You may not need all these to get it, but this is every option I have found the go as far as possible. Also keep in mind that you don't have to be on the extreme of the ugly bar, but just into the red.
***Mistpeak Valley Door:
-->Coop with another hero (can be done local with second controller, see below), and either propose marriage, or do all the available positive interactions (which requires at least one player have the required expression chests unlocked in the road to rule), in front of the door.
---->I did this one personally offline, with my primary profile having all the expressions unlocked in the Road to Rule, and the second account being literally brand new. All positive expressions were initiated by my primary profile until all of them were done, then I did the same with the fresh profile (which had different expressions as it didn't have the expressions unlocked).
***Brightwall Door:
-->You must conceive a child (or adopt), and perform a positive interaction with them in front of the door.
***Sunset House Door:
-->You must simply become King/Queen.
***City of Aurora Door:
-->You must have either 100% good or evil morality. Your choices throughout the game are the primary factors here, so taking the middle ground is a bad idea. Also, to accumulate this over time, adjust the rent and prices for all the properties you own (lower for good, higher for evil). Or, if you have a large quantity of gold, you can transfer that to the castle treasury in the achievements section of the sanctuary (or in the castle itself).
***Mourningwood Door:
-->You must have a level 5 ranged (gun) or melee skill, by upgrading on the Road to Rule.
### To speed up this process, you can typically highlight the Demon Door on the Sanctuary map and then hit fast travel to transport close to the doors, instead of running all the way from the entrances to those regions.
### If you completed the game without enough money to save everyone, and so you're missing merchants, let the game run for a few hours, and the people should return eventually

Thank you for the guide. It was very useful. The one thing I would like to point out is you can't do local co-op for the Mistpeak Valley door. I tried this with my boyfriend and we ended up having to do online co-op.

I did that one on local with a second controller and second account. I'll update the solution to be more clear though.

A little note to help those suffering from the glitch.
When you do the demon doors open the DLC demon door first. That way the counter for the achievement will register the demon doors starting from the problem door.
Plus if you do this then opening the others is easy as you will by then have a level 5 weapon, be a monarch and also reached 100% purity or evil to open the ones in mourning-wood, aurora city and sunset house.

For those experiencing glitches & have a secondary character...
I glitched on mary's books and demon doors -
1) i had no trouble with glitches until mary's books - i got one during battle and collected books with no achs - i loaded 2nd character & replayed collecting her books during inital run at mansion - ach unlocked
2) went back to primary character, finished Lt Huxley's quest, got prisoner clothes for keep demon door - opened & no ach
3) deleted DLC, & cleared cache - downloaded DLC & update again
4) loaded 2nd character (had millsfield & Tr. Keep demon doors to open), finished Traitors keep quests, & 1st 2 quests for the Lt at the Keep, once i got key for his 3d quest, got prison top and opened keep demon door (registered 6/7)
5) got fat and wore multi colored pjs (opposite gender) and unlocked door - registered 7/7 & ach unlocked - no further glitches
Cannot be sure if clearing DLC and cache really helped or not. Understand this is not a straight solution, only a possible fix if you have glitched - If you vote negative please comment as to why. hope this helps you if you are glitching

I'd like to add something what can help you with opening the Demon Doors. I've read a lot about opening DD and these are my ideas. For me, this method worked perfectly, but I can't be sure that the glitch doesn't appear for someone else.
Traitor's Keep downloaded before starting the game at all. Door opening order: Mistpeak Valley, Brightwall, Sunset House, Mouringwood, City of Aurora, Ravenscar, Millfields. The only DD, I've opened in multi was Mistpeak Valley. All others in single player. Traitor's Keep DLC played by myself at all. All DD except Mistpeak Valley opened in single run.
After opening every DD (before going in), I've waited a little bit to see the counter and I saw something strange one time. During opening Mouringwood DD (my 4th), I haven't seen a counter (potential glitch???), so I've quitted to dashbord without saving the game. I've started the game once again and then opened this door again. The counter appeared, so I've gone through it (autosave). That was the only problem with it.
My advices:
1: Open Mistpeak Valley DD first. Open all the next DD alone.
2: Play whole DLC alone. Activate (talk with) Ravenscar DD during DLC (without opening yet)
3: After opening any door, DO NOT enter it if you haven't seen the counter. Quickly dashboard the game (no save) and try again until proper counter appears.
4: Stay in shining area near door until it'll open.

Glitch info, seeing as no other solution on here has this correct:
The Demon Door counter will almost certainly glitch and reset if you play the Traitor's Keep DLC in co-op. I believe this also includes buying the DLC but not actually playing it - since simply buying the DLC brings up the "Set Them Free" counter on your game. Upon starting my game after buying the DLC I had a counter appear with 1/7, which quickly cycled through to 6/7 since I'd opened every main game door. From checking out other players' occurrences of the glitch I'm fairly certain playing co-op messes that counter up.
To stop this - either hold off buying the DLC whilst you do the co-op achievements, or leave the co-op achievements until you've opened every door. Naturally this makes the two-hero door (Mistpeak) hard work though - KKMP's solution says he opened this first which could be a workaround. I had opened that with a guest account on my console, but I hadn't downloaded the DLC before opening it either.

After playing this game through 3 times and being about 30 seconds away from ripping the game out of my xbox smashing it, placing it into a small plastic bag, taking a Taco Bell shit in said plastic bag and mailing it to Lionhead studios I've figured out how to get around the glitch. The glitch is triggered by a co-op player joining your game and resetting the counter.
1. Make sure you have everything you need to open all of the demon doors before you start to open them. i.e. prison outfit, being fat, funky outfit, and 100% good or evil.
2. Set up a dummy account to use as a second player.
3. When opening a door if you don't see the counter pop up quit to the dashboard and reload. Do not change zones, enter the demon door or do anything that would trigger an autosave!
4. You need to do the Mistypeak door first!!! Go to the demon door plug in a second controller and start up your dummy account. Start doing the expressions to make the door unlock. The next part is the most important and you need to make sure you do it quickly otherwise it will not count the door. As soon as the door says "Thank you hero or should i say heroes" press start on the second controller and have your dummy account leave the game. If you dont do this fast enough it will not count.
5. Unlock the traitors keep door next and after that you can unlock the rest in any order.
6. Do not have a co-op player join you at any point after Mistypeak.
