This is the achievement for beating the game and getting one of the two endings. After cleansing the last outpost and visiting the shuttle you'll be prompted to return to the refuge and enter the crypt. Once you watch a few cut scenes you'll be prompted to hammer the A button. Do so until the sequence is finished. This achievement will pop.

I need some help. I cleared all the outposts, but when I cleared the last one and returned to the refuge, i didn't release the crypt to finish the game. What do you still have to do? I've returned to he shuttle several times and returned to the refuge, but I can not see the final cut scenes.

Once you visit Cargo Ship & Shuttle you will be prompted by the game to go back to the initial crypt to finish the game. There you will have two choices for the two endings.
IMPORTANT:
If you want the trophies to register than do not skip the credits until they do.
