How to unlock the Close To Home achievement in Fallout 4 - Definitive Guide
This is a simple story progression quest that appears after you complete the main story line of the DLC (or at least the "The Way Life Should Be" route).
Very self explanatory, but I'm only stating this because I was paranoid that I was missing something until it finally appeared at the end.
It has been pointed out by Kennyannydenny that this could be missed if you tattle on Acadia to The Institute or the Brotherhood of Steel. That is a miscellaneous objective and if you do that you're a monster.

This is actually missable, not main storyline. If you choose to inform the Brotherhood of Steel or the Institute about Acadia (misc objectives), Kasumi will be killed. And as far as i know, you cannot get/complete Close to Home when Kasumi is dead. Correct me if i'm wrong though, in my playthrough i saved Acadia, and Far Harbor, and destroyed the Nucleus.

Good point. Never thought about that.

This guide is for using console commands to quickly complete the DLC quest. You must own the DLC, this will not work if you don't.
Press the tilde key (the button to the left of your 1 key on your keyboard) to open the console command parser.
Type: setstage DLC03MQPostQuest 500
The achievement will unlock right after.
NOTE: if this doesn't unlock, change the 500 to 1000 or 3000.

Final quest of the DLC so story related.
Not really difficult for people who want help with anything check below.

Story related, cannot be missed.
Once you've completed "The Way Life Should Be" and/or "Reformation", you will get the quest "Close to Home". Check in with Kasumi and then return back to the Commonwealth and talk to Kenji. Once you do, the quest is complete.
Just a note first of all:
DO NOT complete this quest before finishing all over trophies, this trophy locks you out of one or two of the questline trophies for whatever reaaon.
Now, there are two ways this can end
This is the final trophy of the DLC.

After dealing with the DiMA situation at the end of "The Way Life Should Be," you'll be tasked with returning to the Nakano Residence in the Commonwealth and relaying the outcome of the island's events to Kenji. A short task, but once he is filled in, the case will close and the achievement will unlock.

The quest appears if you choose to turn off the power of the turbines. If you choose to launch rockets and kill the children of the atom, the quest will not be activated. At first I cut out the children of the atom, I didn’t see the quest. I thought it was a glitch or something. Then I started replaying (fortunately I always save key points on a flash drive) other options, and after turning off the turbines the quest was activated.

It’s possible another way... I blew up the children of the atom, accused Dima of murder, but didn’t tell anyone... and the quest “family matter” dropped out...
