Wrap up the "Reunions" quest to pop this one, it starts after you've finished "Unlikely Valentine".
Details contain a lot of spoilers, proceed at your own risk.
You start towards this quest after finishing "Unlikely Valentine" by sitting down and telling Nick Valentine the details of how your son was kidnapped.
I would recommend preparing for this one, i ran into a deathclaw early in the quest and you will have to fight a lot of synths as well as Kellogg using a stealth boy (invisibility) when you reach your destination.
First you need to search Kelloggs apartment for clues, follow Nick there. The door has a master lock so if you cant pick that you will need to go talk to the mayor to get the key.
The mayor is not just going to give you the key, you will need to pass a speach persuasion, i would recommend saving here and reloading if you fail it. Should you fail you can reload your save, or go talk to his secretary Geneva and try to pursuade her to give you the key. You can also get Kelloggs key from the Mayors safe in his office, either picklocking (advanced) or hacking his computer (novice) and unlocking it. When you have it, return to Kelloggs apartment.
Once inside the apartment, go behind the desk and press the button under it, it will open up a hidden room. Pick up the cigars and talk to Nick, tell him to use Dogmeat. After deciding who will come with you (or not) exit the apartment and show the cigar to Dogmeat by talking to him.
You will need to follow Dogmeat as he tracks Kellogg. He will stop a few times and require you to give him a new scent to follow, which will be either a cigar (San Francisco Sunlights) or bloodied bandages. Just interact with them and follow Dogmeat until you reach Fort Hagen. Look out for the turrets here.
The main entrance is blocked, instead follow the building to the left and then enter the first door to the right and go down some stairs. Down here you will find a door leading into Fort Hagen.
Just make your way through the building, fighting synths and the forts security, until you can confront Kellogg. There will be a conversation that ends with him and two synths attacking you. This fight can be challenging so saving is a good idea.
When Kellogg is dead you will need to grab his password as well as the cybernetic brain augmenter from his body (this is needed for another quest), then use his terminal in the same room and read his logs.
Now just talk to Nick and Piper (at least one of them should be in Nicks office in diamond city, depending on who you had as your follower), it will then complete "Reunions" and start the next quest "Dangerous Minds" as well as pop the achievement.
Fallout 4 can be a bit buggy in general so always SAVE OFTEN.

You can get Kelloggs key from the Mayors safe in his office, either picklocking (advanced) or hacking his computer (novice) and unlocking it. :)

I tried doing it 3 times and it hasn't unlocked. I also didn't get the level 25 achievement. weird.

Just a tip for everyone as this is a pretty long fetch-quest in an open environtment - if you ever get lost (and you will) use VATS to find dogmeat and continue yuot quest. He can be a real pain in the ass to find sometimes after a fight.

I didn't pick up the suggested item off the floor - is that going to be an issue, or should I reload a save?

@TheIcemanCometh - If you mean the cybernetic brain augmenter then yes, you need that for another quest. But before you resort to reloading a save (if you have progressed) try going back to Fort Hagen and check if you can still pick it up.

@zordnil - Yeah, that's what I wound up doing. I did have one before the fight but didn't want to do it again, so I picked the first one after the fight, and luckily everything was still there. The dialog options with Valentine definitely changed once I had it. I only had to replay maybe 20 minutes, but that's fine.

I can't get that far in the quest one terminal requires master hacker which me being only level 19 yeah can't get through the door...

@SorcerousPancho - Been 2.5 years since i played this but i dont think i had master hacker either. There should be another way around.

Solution is dead on, I just wanted to add something...
I had Nick as my companion & after I killed Kellogg with Nick, I then talked to Nick in Kellogg's "office" & the achievement didn't pop. Nick mentions Piper might know more, so I traveled to Diamond City to speak with her. After choosing who to take, Piper or Nick, it then completed the mission, the achievement popped & then started the mission "Dangerous Minds".

@The Snapple Cap - Sorry to hear that, can you reload a save at least? Was it something specific that glitched and made the quest impossible to complete or did the achievement just not pop? Maybe someone reading here knows of a way to solve it.

@The Snapple Cap - Dont know what to tell you except keep trying. You could keep a save before doing that quest and come back to it at a later time, maybe they fix the problem in a patch. Or it might just pop while doing something else, i had a comment on my solution for "War Never Changes" saying that achievement popped for him 17-18 hours into the game when it is supposed to pop in the beginning.
Once you finish the quest "Getting a Clue" you will start the quest "Reunions"
After you kill Kellog you will see the Brotherhood of Steel ship fly above wait till you receive the quest "Shadow of Steel" and then climb down the building. Then fast travel to Diamond City and speak to Piper to finish the quest

After getting help from Nick Valentine you will follow Dogmeat to Fort Hagen, this is where you suspect Kellogg to be hiding. Traverse through the building, taking care of all the Synths until you reach Kellogg. Talk to him then you will have to fight him, he isn't too tough to kill but will throw grenades and go invisible a lot.
