How to unlock the Vault Dweller achievement in Fallout 4 - Definitive Guide
Then grab the Pipboys and Vault Suits from the crate and give them to Clem.
Some poeple say it's glitched. Give the pipboy first then the vault suit and equip the suit.
See the video for example you can't go wrong.

There are ghouls, so you may want to:

Head to the marker underground which takes you inside. There are ghouls trying to break into the vault, kill them and enter.
On your right when you enter the vault there is a container with Vault 88 jumpsuits and Pipboys inside. Take them.
There are ghouls inside the vault, kill them and follow the instructions to get inside. You'll need to activate a terminal, take a control board from a ghoul, and activate a workshop.
Then spawn a settler:

Use the workshop editing mode to assign them to Vault 88.
Talk to them to trade items, give them a Pipboy (they will equip it automatically) and a Vault 88 jumpsuit. You must equip the jumpsuit when it is in their inventory.
I did it a little out of this order and spoke to the Overseer first, which I don't assume is necessary.
Edit: I was in the Workshop menu and found a Store (i.e. drink stand) then while that was up on my screen I went and found the settler and hovering over the settler was able to choose Command/Move options
*WARNING* This achievement seems to have glitched for a lot of people the best way I found to make sure it doesn't glitch is by giving him the pit boy first and make sure its only one, the give him the suit again making sure to only give him one, and then equip the suit and leave the trade screen. I can confirm my game did glitch at first but doing it that way allowed me to get the achievement. Just to be sure I would make a save before giving him any items.

After entering the vault gate you’ll see a blue stash box on the right with jump suits and pip-boys in the very first room with a ghoul in it.
I gave one of each to Piper then equipped the suit on her and the Achievement unlocked.
Only issue is the Pip-Boy disappears in her inventory afterward. There was no way to get it back.
The easiest dweller to do this with is obviously Clem since giving him the vault suit is a requirement. Put both the vault suit and the pip boy in his inventory. The pip boy will be automatically equipped, but you will have to make sure he equips the suit. Once he has both items in his inventory and is actually wearing the vault suit, you'll get this achievement.
After interviewing Clem, you will be given the quest "Power to the People" and will initially be asked to put him in a Vault suit. The Vault 88 suits and Pip-boys can be found in the same blue crate near where the interview was conducted (which is to your right from where you entered the vault initially).
To equip them on a "settler" (e.g. Clem) simply talk to one and a trade screen will pop up. Give them the suit and Pip-boy. Note that after giving them the items, you also have to equip the suit with whilst in the trade screen and selecting the suit.
After completing "Explore Vault 88," you should continue working on "A Model Citizen." During this quest you will conduct three interviews with Overseer Barstow for potential Vault residents. Once the final interview is done, grab the Vault 88 Jumpsuit on the nearby trunk and check inside said trunk for additional suits and Pip-Boys.
You can now speak with your new resident, Clem, and ask to trade items. Here, you can give him a Pip-Boy (found under your Misc. tab) and a Vault 88 Jumpsuit (found in the Apparel tab). Make sure to actually equip the Jumpsuit and the achievement will unlock.
